  2. Transferees (Incoming Gr. 8, 9 & 10 from other schools)
  3. All Incoming Grade 11 students (Grade 10 completers of SLSI & from other schools)

CONSENT IS GIVEN FOR THE FOLLOWING

  1. Collection of:
    • learner data which would include personal, family, health, school information and academic performance results
    • learner picture
  2. The data gathered are used for the following purposes:
    • verification process for admissions examination
    • utilized for admissions information dissemination and orientation
    • determining the admissions and enrolment status of the learner
    • processing the learner information system (LIS)
    • processing and documenting the E-GASTPE grants: JHS Education Service Contracting (ESC) and Senior High School Voucher Program (SHSVP)
    • protection of the health and vital interest of the learner through medical or health evaluation
    • utilized for emergency reasons
    • school publication, website and school social media platforms, if chosen
    • utilized for overall school information data gathering
  3. Disclosure. The learner’s data are shared to the following:
    • The school administration (school directress, assistant directress, and principal)
    • School Registrar
    • Teachers
    • Counselors
    • School Librarians
    • School Nurse
    • Learner Financial Assistance Committee (LFAC)
    • Department of Education (DepEd)
    • Private Education Assistance Committee (PEAC)
    • Catholic Education Association of the Philippines (CEAP)
    • Other government and non-government organizations, if needed, and upon approval of the administration
  4. Storage. Physical documents collected are stored in secured locations
  5. Disposal. All data pertinent to the application for admission are stored in the Registrar’s Office and Guidance and Counseling Office
    • Documents of learner applicants who are not accepted have not enrolled are kept within the year. Documents submitted may still be retrieved by the parent/legal guardian within this period. Upon the laps of 12 months after the start of classes, print documents shall be shredded for proper disposal.
  6. Release of Student Information

    Aside from those stated in this Data Privacy Consent Form, I hereby voluntarily and knowingly authorize any of the following representatives to process the issuance/release of my child’s/ward’s records:

VERIFICATION/AUTHORIZATION

Saint Louis School, Inc. (SLSI) is committed to protect the privacy of the information written herein as required by the Data Privacy Act of 2012. As such, SLSI needs to obtain the parent’s/ legal guardian’s written consent before the application will be processed.

As the parent/ legal guardian of the learner applicant, I have carefully read the information needed in this form. Any falsification, misrepresentation or withholding of information requested from this form will automatically nullify my child/ ward’s application and enrolment. I understand that I will be liable to the exclusion of my child/ ward from SLSI for any falsification, misrepresentation or withholding of information employed by me or on my behalf in relation to this application.

In so doing, I knowingly and voluntarily give consent to the disclosure and processing of my child/ ward’s personal information contained in this form and other documents related to the application of my child/ ward. I understand that the information may be shared with authorized personnel of the school with legitimate educational interest. This is to also authorize any school that my child/ ward has previously attended to release any information/ records requested by SLSI in relation to this application. SLSI may utilize the information in the processing of the application of my child/ ward. All inactive documents shall be securely discarded one year after the date of application.

I am aware of the services provided by the SLSI Guidance & Counseling Office. As such, I fully give my consent and agree to: (1) participate in the programs and services of the Guidance & counseling Office which need my presence. (2) the counseling services that will be provided to my child/ward. (3) the recommendations from the Guidance and Counseling Office for the welfare of my child/ward.

I give my consent for my child to receive clinic services, which include but are not limited to health education, annual medical/dental check-up, referral to health care providers in the community, nutrition services, and first aid/emergency care. I understand that it is my responsibility to notify the school physician, nurse about changes in my child/ward’s health care. SERVICES WILL NOT BE PROVIDED WITHOUT CONSENT AS REQUIRED BY LAW, EXCEPT FOR EMERGENCY MEDICAL CARE.
